2013 Fellowship Program

Short Term fellowship duration 4 to 12 weeks

Requirement

· Be a member of AOSpine

· Be a graduate of a spine program or be in an orthopaedic or neurosurgery residency for at least two years

· Participated in an AOSpine Course

· Some previous experience of spine related instrumentation

· Should not have participated in any other AOSpine fellowship within the last two years

· Preferably published at least one to two spine/orthopaedic/neurosurgery related articles in peer review journal

· Upon completion of fellowship, the fellow has to submit the following documents within 30 days to receive the fellowship certificate:

· Fellowship feedback form

· Fellowship logbook

· Fellowship report (~500 words) with photos to pose on the AOSpine website

Kindly note that the number of fellowship applications have been doubled over the years, please help us to speed up the review process by organizing the application form and supporting documents in the following order:

· Fellowship application form

· Curriculum vitae

· A list of publications and/or lectures written or delivered by the applicant

· Personal statement

· Copy of medical school diploma

· Copy of an official AOSpine course certificate

· Two referee reports signed by your superiors

· Proof of language skills in English

If you are going to send the application electronically, please combine the above documents in order into one PDF file.

One the other hand, if you are going to send by post, please also combine all documents in order without stable.
